    Ameritox Ltd. has bilked Allstate Insurance Co. out of $1.3 million since 2007 by seeking reimbursement for fraudulent urine drug testing services after using a multistate kickback scheme to rake in a vast supply of urine samples, the insurer told a New York federal court Thursday.
