Safety Lapses Caused Chevron Refinery Fire, Report Finds

Law360, New York (April 16, 2013, 5:23 PM EDT) -- Chevron Corp. failed to adequately address safety hazards at its oil refinery in Richmond, Calif., including ignoring its own recommendations, which led to an August fire that injured five workers and sent thousands of nearby residents to the hospital, federal investigators said Monday.
