STIPULATION AND ORDER OF DISMISSAL WITH PREJUDICE Come now the parties, by counsel, and stipulate that all matters in controversy between the Plaintiff and the Defendants have been resolved and settled. The parties stipulate and jointly request that the Court dismiss all claims made, or which could have been made, by and between the Plaintiff and the Defendants with prejudice, including all claims against CSL Plasma, Inc. and the individual Defendant, Melissa Hart, who was never served with the 12 Amended Complaint.
