MINUTE entry before the Honorable Sharon Johnson Coleman: The Court is in receipt of the Court Monitor's Status Report on the SODC List 973 and grants the relief requested therein. Defendants are ordered to provide the Court Monitor with the guardianship order and/or court order for each LCM removed from Defendants' December 2025 LCM SODC Report. For the LCMs whose legal guardians do not consent to community placement, provide written documentation of the legal guardian's opposition to community placement, including at a minimum, the date upon which this opposition was documented, the entity that obtained the information from the legal guardian, and the script or other information used in the discussion with the legal guardian to ensure the decision was a fully informed decision. In the event the Class Member's preference differs from that of their guardian regarding permanent placement in an SODC, provide written documentation including, at a minimum, the date upon which this difference in preference was documented, the entity that obtained the information from the LCM, the script or other information used in the discussion with the LCM to ensure the decision was a fully informed decision, and documentation that the IDHS conflict resolution process has been fully implemented.
