   ORDER (Approving Wrongful Death Settlement) - It is hereby ORDERED that Friedman Fisher Verity, PLLC be paid $170,000.00 and the Commonwealth Law Group, LLC be paid $170,000.00; and it is hereby ORDERED that Friedman Fisher Verity, PLLC be reimbursed for costs associated with this matter in the amount of $15,379.23, and that the Commonwealth Law Group, LLC be reimbursed for costs associated with this matter in the amount of $19,152.69, which sum is to be paid out of the settlement proceeds in this matter; and it is hereby ORDERED that the remainder of the settlement proceeds be distributed for the benefit of the Statutory Beneficiary in the amount of ($415,000.00) from TDC Specialty Insurance Company on behalf of the Medical Defendants payable to Athene Qualified Assignment Corporation to fund periodic payments on the dates and in the amounts indicated hereafter. It is further ORDERED that Carolyn Arhart Edwards and Misty Arhart Wood are authorized to sign the joinder agreement, attached hereto as Exhibit A, and any other appropriate documents to establish this trust. It is further ORDERED that the Defendants and their heirs, insurers, or reinsurers, their past, present, and future owners, officers, directors, stockholders, attorneys, agents, representatives, employees, independent contractors, subsidiaries, parent companies, partners, predecessors and successors in interest, heirs, executors, administrators, and assigns of any of the foregoing, are released and forever discharged from all liability which may be asserted against any or all of the Defendants arising out of the death of Decedent.
