VA Nurse Faulted For Failing To Resuscitate Patient

By Rachel Graf (November 8, 2017, 9:10 PM EST) -- A Veterans Affairs nurse inaccurately told other staff that a patient had a Do Not Attempt Resuscitation order, and the patient died after the staff relied upon the nurse's statement and did not attempt resuscitation, the Department of Veterans Affairs Office of Inspector General said in a report Tuesday.

A review that began in January found the VA Ann Arbor Healthcare System's medical facility didn't have a "standardized process" for communicating a patient's resuscitation status among staff, resulting in the error. Higher-ups at the facility in Ann Arbor, Michigan, had identified the reporting error about a year before this patient's death...
